A response is submitted through Forms for Slack and the right person may need to act before the message gets buried among everyday channel activity. Forms for Slack's New Form Response trigger can start a Zap and send an SMS through Mobile Message, so a selected manager or team member receives a direct alert. This is useful for urgent requests, approvals and operational forms that need a quick human response.

The trigger can send a single message with Send SMS, or notify a group with Send SMS to List when several people share responsibility. Mobile Message's Inbound SMS Received trigger can also create a contact when someone texts your Mobile Message number, although Forms for Slack has no listed action for writing the reply back into a form. The useful direction here is therefore a response from Slack forms to SMS.

Zapier connects the accounts without code and lets you map the response fields into the text. Forms for Slack remains the place where responses are submitted, while Mobile Message delivers the alert.
